I agree to pay the first months fees which include the Monthly Tuition payment and the Annual Registration Fee of $20.00. I understand that Monthly Tuition is due for each month thereafter until given notice to withdraw my child from the Just 4 Flips program.  Otherwise I understand that my child will continue to attend classes and I will continue to be responsible for tuition payments.

Liability Release:
Your child will remain under the care, direction and supervision of the school while receiving instruction from Just 4 Flips, Inc.  I Herby release and discharge Just 4 Flips and its members from all actions, claims, or demands, I, my heirs, distributees, guardians, legal representatives, or assigns now have or may have in the future for injury or damage resulting from my child's participation in this activity.

(Notice required by USA Gymnastics)- By the very nature of the activity, gymnastics carries a risk of physical injury. No matter how careful the gymnast and coach are, no matter how many spotters are used, no matter what surface exists, the risk cannot be eliminated. The risk of injury includes minor incidents such as broken bones, dislocations and muscle pulls. The risk also include catastrophic injuries such as permanent paralysis or even death from landings or falls on the back, neck, or head.
